Iodinated contrast is a form of intravenous radiocontrast agent (radiographic dye) containing iodine, which enhances the visibility of vascular structures and organs during radiographic procedures. Some pathologies, such as cancer, have particularly improved visibility with iodinated contrast. Patients are frequently asked about iodine or seafood allergy before IV contrast material is administered because of a commonly held belief among radiologists and others in the medical community of a specific cross-reactivity between iodinated radiographic contrast material and other iodine-rich substances. "Iodine allergy" is often used as a collective term for adverse reactions to these agents.
Patients receiving contrast media via IV typically experience a hot feeling around the throat, and this hot sensation gradually moves down to the pelvic area. Intravenous (IV) dye (dye given through the vein)—also known as radiocontrast media (RCM)—is used widely in the United States for various radiological studies, such as angiograms, X-rays, magnetic resonance imaging (MRI), and computed tomography (CT) scans. Adverse reactions to RCM are fairly common, though allergies are rarer. Remember that every administration of a radiopaque contrast agent has the potential to cause an anaphylactic reaction, even if the patient has received contrast in the past with no reported adverse effect. This review will discuss the relationship of radiocontrast, iodine, and seafood allergies. This should hopefully put to rest the myth, that iodine is an allergen and that an allergy to shellfish does not increase the risk of reaction to radiocontrast agents any more than that of having other allergies. The type and amount of radiopaque filler compounded with thermoplastic materials in the manufacture of these devices determines how they appear. The filler affects the degree of contrast and the sharpness of the image to the extent that it influences the attenuation of x-rays passing through the body and the device.

We report an illustrative case of a delayed cutaneous reaction to IC and review the literature on the epidemiology and management of this underrecognized phenomenon. As with all radiopaque contrast agents, the lowest dose of OMNIPAQUE necessary to obtain adequate visualization should be used. A lower dose may reduce the possibility of an adverse reaction. Most procedures do not require use of either the maximum volume or the highest concentration of OMNIPAQUE. An allergic reaction to contrast dye can have serious and sometimes fatal consequences.
